Health and Welfare
Full-time and part-time employees are eligible for benefits in less than 90 days after you start. When it comes to your insurance needs, we know that each one of you has unique situations, which is why we offer a variety of healthcare plans.
LifeTimes, our benefits administrator, will mail a benefits packet to your home with detailed descriptions of each benefits plan as well as instructions on how to enroll for both new hires and open enrollment.
Medical
The multiple medical options allow you to focus on your unique healthcare needs. You choose the coverage most appropriate for your situation. The plans vary from an economic plan designed for emergency healthcare situations to a more comprehensive plan designed to limit your out-of-pocket expenses for all your healthcare needs.

Prescription Drugs
All levels of the medical insurance plans include prescription coverage. Deductibles and copays depend on your level of coverage, as well as type of prescription (name brand, generic, mail order, etc.).
Dental
Good dental health affects more than the way you look. It affects the way you feel and your overall health. Our plan makes it easy for you to afford preventive care such as checkups and cleanings. It also provides benefits to help you pay for other expenses such as fillings, dentures or orthodontia.
Under this plan, you may choose any dentist you like, but you receive provider discounts when you use a MetLife PPO network dentist. Under the Dental PPO plan, preventative care such as check-ups, x-rays and cleanings are covered at 100% of reasonable and customary charges. To receive coverage for major or orthodontic services, you must participate in the dental plan for at least 1 year prior to your treatment. Effective: 1st of month following 60 days of employment.

Vision
The EyeMed Vision Plan covers expenses for regular eye exams, lenses, frames and contacts. When you use an EyeMed provider, you pay a copay or receive an allowance for most covered services and materials. You can also use a non-EyeMed provider but at a reduced benefit.

Life Insurance
Life Insurance coverage is offered to help provide financial security for you and your family in case of death. If you are a full-time employee, your facility provides Basic Term Life coverage at 1 times your annual salary at no cost to you. You may also elect Supplemental Life Term life for 1-5x your annual salary.
Effective: 1st of month following 60 days of employment.
Long-Term Disability
A disability can affect much more than your health. It can change practically every part of your life — including your personal finances. You can purchase Long Term Disability (LTD) coverage to replace part of your income if a serious accident or illness leaves you unable to work. You may elect 50% or 60% of your pre-disability pay which would begin payint out after you have been partially or totally disabled for at least five months.
Flexible Spending Accounts
Flexible Spending Accounts (FSAs) allow you to set aside money from each paycheck — before most taxes are deducted — to reimburse yourself for eligible healthcare and day care expenses. You save money because the taxable portion of your income is reduced. LifeTimes Benefit Choices offers two FSAs:
- Health Care FSA
- Day Care FSA
Contributions you make to the Health Care FSA are taken from your pay before federal income taxes or Social Security and Medicare taxes are deducted. That means the taxes on your remaining pay are lower. You then use these before-tax dollars to reimburse yourself for eligible healthcare expenses during the year.

Core Plus Benefits
After you complete two months of service, you are eligible to enroll in a voluntary benefits program through CorePlus Benefits. CorePlus Benefits are designed to complement and supplement the LifeTimes Benefits plans.
Legal Benefit — Provides access to a nationwide network of over 9,000 attorneys who can provide numerous free and discounted legal services to its members.
Long-Term Care — Assists you financially as you receive or provide long-term care for those you love — perhaps in a nursing home or hospice facility.
Short-term Disability — Replaces a percentage of your income while you are out of work due to a covered illness or injury.
Universal Life Insurance — Combines permanent and portable life insurance protection with the potential to grow cash value.
